A Case of Severe Rebound after Switching from Fingolimod to Ocrelizumab

Multiple sclerosis (MS) is a chronic autoimmune demyelinating inflammatory disease of the central nervous system that primarily affects young people. Since the 1990s, MS has been treated with disease-modifying drugs (DMD). Besides immunomodulators used as the first-line therapy for cases of low disease activity, second- and third-line drugs have been developed in recent years with high anti-inflammatory and specific targeted immunosuppressive effects, such as monoclonal antibodies, for the treatment of high disease activity cases. With the increased number of treatment options, drug switching has become a common situation in clinical practices. DMD may be discontinued due to drug intolerance or side effects, difficulty in their follow-up and use, risk of progressive multifocal leukoencephalopathy, high cost, or pregnancy. Discontinuation of second- and third-line drugs can lead to rebound syndrome with the worsening of clinical and radiological picture, development of resistance to treatment, and progression of the disability. In this article, we present the case of progressive clinical deterioration and treatment resistance caused by a switch from oral fingolimod to intravenous ocrelizumab treatment due to high disease activity. The purpose of the study is to demonstrate the importance of caution while switching from one drug to another.


Fingolimoddan Okrelizumaba Geçiş Sonrası Gelişen Ağır Rebound Olgusu

Multipl skleroz (MS), öncelikle genç erişkinleri etkileyen santral sinir sisteminin kronik otoimmün demiyelinizan ve inflamatuar bir hastalığıdır. Geçen yüzyılın son on yılından günümüze dek MS tedavisinde hastalık modifiye edici ilaçlar (DMD) kullanılmaktadır. Düşük hastalık aktivitesinde kullanılan, birinci basamak dediğimiz immünomodülatörlerle birlikte, son yıllarda, aktif seyir gösteren olgularda, monoklonal antikorlar gibi yüksek bir antiinflamatuar ve spesifik hedefe yönelik immünsüpresif etkiye sahip, ikinci ve üçüncü basamak ilaçlar kullanıma girmiştir. İlaç sayısındaki artış nedeniyle, önceki kullanılan ilacının kesilmesini gerektiren ilaçtan ilaca geçiş sayısı da artmıştır. Ayrıca, gösterdikleri yan etkileri, ilaç intoleransı, progresif multifokal lökoensefalopati gelişme riski, yüksek maliyet, gebelik, takip veya uygulama zorluğu, DMD ilaçlarının bırakma sebepleri arasındadır. İkinci ve üçüncü basamak ilaçlarının kesilmesi, klinik ve radyolojik tabloda kötüleşme, tedaviye direnç gelişimi ve dizabilite ilerlemesi ile seyreden rebound sendromuna yol açabilir. Biz bu yazıda, yüksek hastalık aktivitesi nedeniyle oral fingolimod tedavisinden intravenöz okrelizumab tedavisine geçiş sırasında, ilerleyici klinik kötüleşme ve tedaviye direnç gösteren olguyu, ilaçtan ilaca geçiş sürecinde dikkatli olunmasının önemini vurgulamak için sunduk.
