Assessment of Psychiatric Symptoms and Quality of Life in Individuals with a Diagnosis of Recurrent Aphthous Stomatitis

Objective: Recurrent aphthous stomatitis (RAS) is a disease that is characterized by painful and recurrent ulcerations in the oral mucosa. Although its etiopathogenesis is not fully understood, it is believed that psychogenic factors may be associated with aphtha formation. It is known that psychiatric symptoms affect the clinical manifestation of some dermatological disorders. Here, we investigated the effect of RAS on the patients’ quality of life as well as the accompanying psychiatric symptoms in individuals diagnosed with RAS (Recurrent Aphthous Stomatitis).

Materials and Methods: 35 individuals who applied to the dermatology outpatient clinic and who were diagnosed with RAS were recruited as the patient group. Fifty healthy sociodemographically-matched individuals were recruited as the control group. A sociodemographic data form prepared by us, the Beck Depression Inventory (BDI), the State Trait Anxiety Inventory There are 2 forms of this scale and both were used in our study: Stai-1 and stai-2 (STAI 1/2), and the World Health Organization Quality of Life-Short Form (WHOQOL-BREF-TR) were administered to all participants.

Results: The average BDI and STAI 1/2 scores of the patient group were significantly higher than the scores of the control group. In addition, the total and subdimension scores of WHOQOL-BREF-TR of the patient group were significantly lower than those of the control group.

Conclusion: The statistically significant difference observed ­between the patient and control groups supports the hypothesis that psychological stress plays a role in the pathogenesis of RAS. Therefore, it will be beneficial to add supportive psychiatric treatment for RAS at the onset of symptoms because RAS negatively affects the patients’ quality of life owing to its pain and severity.

 


 

Rekürren Aftöz Stomatit Tanılı Bireylerde  Psikiyatrik Belirtiler ve Yaşam Kalitesinin Değerlendirilmesi

 

Amaç: Rekürren aftöz stomatit (RAS), ağız mukozasında ağrılı ve tekrarlayıcı ülserasyonlarla karakterize  bir hastalıktır. Etiyopatogenezi tam olarak bilinmese de psikojenik faktörlerin aft oluşumuyla ilişkili olabileceği düşünülmektedir. Ruhsal belirtilerin bazı dermatolojik hastalıkların kliniğini etkilediği  bilinmektedir.  Biz rekürren aftöz stomatit tanılı bireylerde eşlik eden psikiyatrik belirtileri ve bu durumun yaşam kalitesi üzerine olan etkisini araştırdık.

Gereç ve Yöntem: Dermatoloji polikliniğine başvuran ve  rekürren aftöz stomatit  tanısı konulan  35 birey  hasta grubu olarak alındı. Hasta grubuyla sosyodemografik açıdan benzer özellikleri taşıyan 50 sağlıklı birey ise kontrol grubu olarak belirlendi. Tüm katılımcılara tarafımızca hazırlanmış Sosyodemografik Veri Formu ile Beck Depresyon Ölçeği (BDÖ), Durumluk Sürekli Kaygı Ölçeği (Stai 1/2), Dünya Sağlık Örgütü Yaşam Kalitesi Ölçeği-Kısa Formu Türkçe Versiyonu (WHOQOL-BREF-TR) uygulandı.

Bulgular: Rekürren aftöz stomatit tanılı bireylerin BDÖ, durumluk ve sürekli kaygı puanları ortalaması, sağlıklı kontrol grubuna göre anlamlı şekilde yüksek bulundu. Ayrıca RAS tanısı alan hasta grubunun WHOQOL- BREF-TR ölçeğinin toplam puan ve alt boyutlarının puanları kontrol grubunun puanlarından anlamlı şekilde düşük saptandı.

Sonuç: Rekürren aftöz stomatit tanısı olanlar ile kontrol grubu arasında gözlenen istatistiksel düzeydeki bu anlamlı farklılık, psikolojik stresin RAS ‘ın patogenezinde rolü olabileceğini desteklemektedir. Ağrı ve şiddeti ile hayat kalitesini olumsuz etkileyen RAS tedavisine psikiyatrik tedavi desteğinin eklenmesi RAS’ ın başlangıcı ve semptomları üzerine faydalı olabileceğini düşündürmektedir.
