Comparison of Treatment Results of Stable Intertrochanteric Femur Fractures with Dynamic Hip Screw and Proximal Femoral Nail

Objective: According to the development in human life conditions, life time expectations are prolonged and proximal femoral fractures are increased. In this study, we had evaluate and compare the clinical and radiologic parameters in which we treated stable intertrochanteric femur fracture with dynamic hip screw (DHS) and proximal femoral nail (PFN).

Method: Patients were evaluated retrospectively in which were treated due to per-trochanteric femur fracture. Femoral head or neck fractures, subtrochanteric fractures, posteromedial cortex communited fractures and reverse oblique intertrochanteric fractures were excluded. 63 patients whom treated due to stable intertrochaneric femur fracture were included. There was not age limitation. Patients were evaluated with plain radiographs and Harris Hip Score (HHS). Statistical differences between DHS and PFN group according to patient age, surgical time, age, gender, fracture side, fracture type, anesthesia type, pre-operative risk assesment (ASA) type, surgical time, post-operative fracture distance, post-operative reduction quality, healing time and HHS were searched. Statistical significance value was taken as P < ,05.

Results: There was not statistically difference in collo-diaphyseal angle in fracture side and unaffected side either in DHS group and in PFN group (P > ,05). There was not statistically difference in Garden alignment index in fracture side and unaffected side either in DHS group and in PFN group (P > ,05). Total blood tranfusion amount was higher in DHS group than PFN group (P < ,05). Intra-operative bood transfusion is higher in DHS group than PFN group (P < ,05). Post-operative blood tranfusion rate was similar in DHS and PFN group (P > ,05).

Conclusion: Both DHS and PFN had shown fine results with acceptable complication rates. Both treatment modalities can be used safely in stable intertrochanteric femur fractues, clinically and radiologically.
