Which Method Is More Satisfying for Patients? Comparison of Primary Repair and Limberg Flap Methods in Surgical Treatment of Sacrococcygeal Pilonidal Sinus: A Retrospective Clinical Study

Objective: Pilonidal sinus (PS) is an acute and chronic infectious disease that commonly occurs along the natal cleft line in the sacrococcygeal region. Several surgical techniques and medical strategies are used for the treatment of this disease. This study aimed to present the follow-up results of patients with PS treated with two different surgical techniques that are commonly used at our clinic.

Materials and Methods: As part of the treatment for PS, primary repair (Group 1, n=153) and excision+Limberg flap technique (Group 2, n=159) were performed on the patients. All the patients were retrospectively examined based on the following parameters: age, sex, operation time, hospitalization duration, surgical site infection development, wound separation and necrosis, long-term recurrence, local discomfort, and loss of sensation in the wound area.

Results: There was no difference in terms of demographic features, wound separation, and recurrence between the surgical techniques employed (p>0.05); however, it was observed that the operation time, hospitalization duration, local discomfort, and loss of sensation were decreased and patients were more comfortable in Group 1 (p<0.05).

Conclusion: Primary repair should be preferred for the surgical treatment of uncomplicated PSs because it causes less discomfort and loss of sensation and results in shorter operation time and hospitalization duration. Moreover, this technique is not significantly different from the Limberg flap technique in terms of surgical site infection, wound separation, and recurrence rates. However, surgical treatment with closure techniques, such as the Limberg flap technique, should be preferred in cases where a wide area is affected.

 


 

Hastalar Hangi Yöntemden Daha Memnun? Sakrokoksigeal Pilonidal Sinüsün Cerrahi Tedavisinde Primer Onarım ve Limberg Flep Yöntemlerinin Karşılaştırılması: Retrospektif Klinik Çalışma

 

Amaç: Pilonidal sinüs (PS) sakrokoksigeal bölgenin natal cleft hattı boyunca sık görülmekle birlikte akut ve kronik şekli olan infektif hastalığıdır. Hastalığın tedavisinde birçok cerrahi ve tıbbi teknik kullanılmaktadır. Bu çalışma ile bir devlet hastanesinin genel cerrahi kliniğine başvuran pilonidal sinüs hastalarının tedavisinde sık kullanılan iki farklı yöntemi inceleyeceğiz.

Gereç ve Yöntemler: Hastalara pilonidal sinüs tedavisinde Primer Onarım (Grup 1, n=153), eksizyon ve Limberg flep (Grup 2, n=159) cerrahi teknikleri uygulandı. Tüm hastalar demografik olarak yaş, cinsiyet, operasyon süreleri, hastanede yatış süreleri, cerrahi alan enfeksiyonu gelişmesi, yara ayrılması ve nekroz, uzun dönemde nüks, yara bölgesinde lokal huzursuzluk ve his kaybı açısından incelendi.

Bulgular: Araştırma sonucunda uygulanan cerrahi yöntemler arasında hasta tanımlayıcı özellikleri, yara ayrılması ve nüks açısından fark olmadığı saptanırken (p>0,05), Primer Onarım uygulanan grupta operasyon sürelerinin, hastanede kalış sürelerinin, lokal huzursuzluk ve his kaybının daha az olduğu ve daha konforlu oldukları tespit edildi (p<0,05).

Sonuç: Komplike olmayan pilonidal sinüslerin cerrahi tedavisinde daha az huzursuzluk ve his kaybı, daha az ameliyat süresi ve hastanede yatış süreleri ve Limberg flep tekniği ile arasında cerrahi alan enfeksiyonu, yara ayrımı ve nüks oranları açısından belirgin fark olmaması nedeniyle primer onarım tekniği tercih edilmelidir. Ancak, özellikle geniş bir alanı etkileyen vakalarda Limberg flep gibi kapatma teknikleri ile cerrahi tedavi tercih edilmelidir.
