Management of Gingival Recession Around the Prosthesis

Gingival recessions around the teeth and fixed prosthesis can occur due to many factors. The fixed prosthesis made without observing the biological rules may cause gingival recession with the effect of oral hygiene habits. In this case report, we aimed to present the gingival recession of the fixed prosthesis of the upper left lateral tooth with the connective tissue graft. A 23-year-old male patient was admitted to the Department of Periodontology of Oral and Dental Health Training and Research Hospital of Erzincan Binali Yıldırım University with the complaint of bleeding and gingival recession in the gingiva of the fixed prosthesis between maxillary anterior lateral teeth. In the examination, it was observed that the prosthesis did not have marginal adaption with the gingiva and caused gingival recession. The incompatible prosthesis was removed and the areas contacting the gingival margin were shortened by 2 mm and delivered to the patient after the initial periodontal treatment. After the tissue healing was completed, the gingival recession in the lateral tooth was repaired by a connective tissue graft that was placed in via a modified tunnel technique. After waiting for 3 months, new prosthesis were delivered to the patient. At the end of the first year, no pathology was found around the patient’s prosthesis. Especially prosthesis in the esthetic zone to be made in harmony with the gums in accordance with the biological and aesthetic principles can prevent gingival recessions and pathologies of those may occur in the next periods.

Dişeti çekilmeleri doğal diş ve sabit protezler etrafında birçok faktörün etkisiyle oluşabilir. Biyolojik kurallara uyulmadan yapılan sabit protezler zamanla hastanın da oral hijyen alışkanlıklarının etkisiyle diş eti çekilmelerine sebep olabilir. Bu olgu sunumunda sol üst lateral dişteki sabit protezin bukkalindeki dişeti çekilmesinin bağdoku grefti ile onarımını sunmayı amaçladık. Yirmi üç yaşındaki erkek hasta maksiller ön lateral dişleri arasındaki sabit protezinin dişetlerindeki kanama ve dişeti çekilmesi şikayetiyle Erzincan Binali Yıldırım Üniversitesi Ağız ve Diş Sağlığı Eğitim ve Araştırma Hastanesi Periodontoloji Anabilim Dalına başvurdu. Yapılan muayenede yapılan protezin dişetleriyle marjin uyumu göstermediği ve buna bağlı dişeti çekilmesi olduğu görüldü. Uyumsuz protez çıkarılıp dişeti marjinine temas eden bölgeleri 2 mm kısaltılıp başlangıç periodontal tedavisinden sonraya hastaya teslim edildi. Doku iyileşmesi tamamlandıktan sonra lateral dişteki dişeti çekilmesi modifiye tünel tekniğiyle yerleştirilen bağdoku grefti ile onarıldı. 3 ay bekledikten sonraya hastaya basamaklı kesim yapılarak yeni protezleri teslim edildi. Birinci yılın sonunda hastanın protezleri etrafında herhangi bir patoloji tespit edilmedi. Özellikle ön bölgede yapılacak olan protezlerin dişetiyle uyum içinde biyolojik ve estetik prensiplere uygun yapılması sonraki dönemlerde oluşabilecek dişeti çekilmeleri ve patolojilerini önleyebilir.
